INSTRUCTIONS:
From this window you can input the information necessary for ordering a registered copy of the E-Z Credit Kit. Use the mouse pointer (or TAB/arrow key combination) to move to the various areas of the order form and fill it out completely.
* Especially important is the name and address areas as well as the Invoice section.
When your sure all the information is accurate and complete select the PRINT button at the bottom/right of the screen. Once your form is printed, simply enclose the proper payment and the order form in an envelope and mail it it the address at the top of the form.
For those who do not have a printer or are having problems printing then hand write your request with all the information on this screen included on the request.
